I and all individuals of my group/family members, Undersigned participant, intending to be legally bound, do hereby for myself and my group individual assign, forever waive, release and discharge any and all rights, claims and actions for damages that I/we may have, or that may hereafter accrue to me/us against the Patel Samaj Edmonton, their Executive Committee, Directors, members and volunteers. I/We attest and verify that me and all the individuals of the group are physically fit and able to participate in this event and acknowledge that we are aware of the inherent risks in participating in this type of event.
Insurance: Further me and my individuals of the group understand that Patel Samaj Edmonton does not assume any responsibility for or obligation to provide me or my group individual with financial or other assistance, including but not limited to traveling, medical, health or disability benefits or insurance of any nature in the event of injury, illness, death or damage to our property.
Medical Treatment: Medical Treatment: All the individuals of my group/family members hereby release and forever discharge Patel Samaj Edmonton from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during this event.
Photographic Release: All the individuals of my group/family members grant and convey to Patel Samaj Edmonton all right, title, and interests in any and all photographs, images, video, or audio recordings of me or my group individual or our likeness or voice made by Patel Samaj Edmonton.
By signing below, I on behalf of myself and all the individual/family members of my group/family express our understanding and intent to enter into this release and waiver of liability willingly and voluntarily.
